This PR renames the internal namespace to Mathlib.CrossRef, factors
out a shared addCrossRefDoc helper, and replaces hardcoded database
strings with databaseName/databaseURL lookups so further databases
can be added with minimal duplication. Drive-by: #kerodon_tags now
prints "Kerodon Tag" instead of "Stacks Tag".
